  • The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput, improve care quality and efficiency, and ensure efficient resource utilization.
  • The PCRM collaborates with physicians, nurses, and other health care workers to manage patient transitions and serves as the central information source for third-party payers, communicating the plan of care and medical necessity.
