Job Summary
The Social Worker is responsible for the development, implementation and delivery of a high-quality social work service to patients and families across GEM@Home and Rehab@Home.
The GEM@Home and Rehab@Home service is a bed substitution model, replicating bed-based services in the patient’s home where appropriate.
This role involves managing a caseload, incorporating the proficient use of clinical skills, evidence-based practice, effective communication and collaboration.
