The role supports the delivery of CMS clinical care requirements while coordinating care for patients enrolled in the Guiding an Improved Dementia Experience (GUIDE) Model Program.
Candidates will provide comprehensive assessments, develop personalized care plans, and act as a liaison between patients, caregivers, and medical providers across three Delaware locations.

Knowledge of Advance Care Planning and Medication Reconciliation
Strong communication skills for caregiver education

Physician office experience preferred
BSN degree or commitment to complete within 3 years
Experience with Joint Commission tracer monitoring
