Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks PDPM tools utilization.
  • Ensures accurate and timely completion of all MDS Assessments, coordinating interdisciplinary participation and maintaining schedules.
  • Responsible for all level of care changes within the facility, including generating forms and transmitting them to the appropriate agency.
