Job Summary
Coordinate and manage discharge planning, complete resident assessments and related documentation, and schedule and coordinate care conferences.
Participate in MDS-related processes and communicate clearly and compassionately with residents and families.
Collaborate with the interdisciplinary team to support resident needs and maintain accurate, timely documentation.
