Job Summary
The ACT/FACT Housing Specialist provides supportive services including outreach, monitoring, assessment, service planning, support network building, consultation to families, side-by-side skills training, and instruction in the use of community resources.
Job duties include assisting participants with daily living, budget planning, housing maintenance, accessing benefits, meal planning, transportation, and accompanying clients to appointments.
The role involves providing on-call crisis services, completing documentation in the electronic health record, and working a rotating schedule including weekends, evenings, and holidays.
