Job Summary
The primary purpose of this role is to assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to meet residents' emotional and social needs.
Essential duties include assisting with resident self-care, independence, and well-being, as well as providing medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being.
The position involves working with emotional problems, assisting residents and families with anxieties and stress, and coordinating social service activities with other departments and community agencies.

Key Requirements

Must occasionally lift and/or move up to 25 pounds
