Job Summary

  • The primary role is to fulfill patient care by working collaboratively with the patient and treatment team to allow patients access to care that is within the allowable limits for transportation to and from programming.
  • The van driver will be required to travel outside of the building for work related purposes, develop transportation routes, and engage in professional interactions with patients, families and possible referral sources/community partners.
  • Documentation of these services will be required in accordance with necessary State, Joint Commission, and/or CARF accreditations.
