Job Summary
Competently interpret the patients’ history and baseline level of function and then to complete an assessment appropriate to the patient's areas of deficits.
Based on problems identified, the speech language pathologist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff.
There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.
