Job Summary

  • Competently interpret the patients’ history and baseline level of function and then to complete an assessment appropriate to the patient's areas of deficits.
  • Based on problems identified, the speech language pathologist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff.
  • There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

  • Master's Degree
  • ASHA CCC-SLP
  • BLS - Basic Life Support
  • valid state driver’s license
  • acceptable Motor Vehicle Report (MVR)
