Job Summary

• The Integrated Nursing Case Manager provides care/service safely and efficiently for a full range of services to patients of all ages and their families, collaborating, communicating, and facilitating coordination of services during hospitalization and post-hospitalization.
• Primary role is to concurrently facilitate proactive patient care services to ensure that care is appropriate, timely, and cost-effective and achieves the desired outcome, while promoting effective utilization and monitoring of health care resources.
• Responsibilities include conducting admission and continued stay reviews, facilitating external payer certification, coordinating care progression, and facilitating discharge planning, resource referral, and patient education.
