Job Summary
The Pharmacy Senior Clinician undertakes pharmacy specific assessments, plans and implements strategies to support appropriate and quality use of medicines.
Intervention is undertaken in the clients home and community, and secondary consult is an integral part of the role, providing advice and support to team members on medications and their management.
This position will be part of the HIP Complex Care and Chronic Disease Service (CC/CD) aiming to improve health outcomes for people with complex medical and/or social issues.

$108,820.40 - $123,905.60 per annum

Key Requirements
Bachelor of Pharmacy or higher degree
Current AHPRA registration
Registered (unrestricted) with Pharmacy Board of Australia
Victorian Driver’s License
Postgraduate qualification in Hospital/Clinical Pharmacy
