Key Requirements
Current Registered Nurse (RN) license
Bachelor of Science in Nursing (BSN) required or to be obtained within 4 years
Clinical nursing experience in chronic disease management
Basic computer skills and Microsoft Office proficiency
Current BLS certification for home/community visits
Valid driver’s license, auto insurance, and reliable transportation
