  • The Social Worker I is consulted to provide assessment and referral for services related to the complex care needs of the patient and family for adjustment to illness, crisis intervention and to preserve patient's self determination in the decision making process.
  • Conducts a comprehensive assessment of the patients/families behavioral, psychological and socioeconomic needs and identifies resources needed to adapt to illness throughout the continuum of care.
  • Documents measurement and evaluation of social service consultation and satisfaction outcomes as patient progresses towards desired goals.
