• We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients.

• Board Certified
• valid NP or PA certification in Georgia
• 1-2 years of primary care experience
• geriatric patients experience
