Job Summary

• The Case Management Extender PFK helps navigate and access community services and other resources and provides support through maintaining population health programs and care coordination activities.
• The Case Management Extender collaborates to arrange for or connect patients to needed services and identifies, creates, and nurtures relationships with local agencies, schools, churches, and other programs.
• Case Management Extenders are patient and family facing with daily activities during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, community visits, and via telephone.
