Job Summary

  • As a Progression Coach you will participate in the delivery of services to young people, which will include undertaking needs, risk, and vulnerability assessments, developing support and risk management plans, and lead /participate in case review meetings and multi-agency meetings.
  • Support young peoples’ engagement with work and learning, develop positive family and other support networks, assist young people to maintain accommodation and to provide support with any problems which may arise.
  • Ensure young people feel supported and encourage them to engage with internal and external services as part of their support.

Key Requirements

  • NVQ level 3 in health & social care or equivalent
  • Working with young people in youth work, residential or social work
  • Experience with payments by outcomes (PBO/PBR)
  • Experience with young people with multiple complex needs
  • Experience of Safeguarding vulnerable individuals
  • Knowledge of current welfare rights & relevant legislation
  • Knowledge of housing legislation
  • Knowledge of children, adults, and associated care acts
