Job Summary
The primary purpose of this position is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs to meet residents' needs.
Employees will coordinate social service activities with other departments and assist residents in achieving the highest practicable level of self-care, independence, and well-being.
The role requires maintaining confidentiality of all resident care information including protected health information and reporting any suspected incidents of unauthorized disclosure.
