Job Summary

  • Provides direct and indirect patient care in the office setting to patients, families and caregivers in the Medical Group practice.
  • Communicates with outpatient clinic physicians and NP/PAs about changes in patient’s status, symptomatology and results of diagnostic studies.
  • Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
