  • To provide a single point of focus for rehabilitation patients as they move throughout the continuum of care to assure smooth, appropriate, coordinated, timely and resource sensitive transitions.
  • This position operates independently, working cooperatively and collaboratively with patient, families’ health care providers, payors, and management.

Key Requirements

  • Graduate from an accredited school
  • Licensed as a Physical Therapist, Occupational Therapist, Speech Pathologist or Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL)
  • Minimum three (3) years of clinical experience
