Job Summary
The Clinical Pharmacist Specialist provides services for patients with medically and socially complex conditions that originated in childhood.
This position is a key clinical role supporting our Specialty Pharmacy program, embedded within the practice working with the Providers.
This position may work remotely up to two (2) days per week.

Key Requirements
PharmD from an ACPE-accredited School of Pharmacy
ASHP-accredited PGY1 Pharmacy Practice Residency or equivalent experience
ASHP-accredited PGY2 Pharmacy Residency or equivalent experience
Board Certification required within 2 years of hire
