Job Summary

• Our employees help children, at-risk adults, families, and individuals navigate social and economic pressures by connecting them to services and experts who support their overall well-being.
• In this role, you would predominantly perform case management duties involving a variety of complex case situations, with a focus on youth aged 18-21 who had prior involvement in child welfare and have opted back in for continued Child Welfare Services through the Foster Youth in Transition Program (FYIT).
