• In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management.
• You’ll work on‑site in clinic, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct in-person and virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.

• Doctor of Pharmacy (PharmD)
• Licensed pharmacist in Washington
• PGY2 Oncology Residency OR PGY1 Ambulatory Care Residency and 3 years experience OR 3 years clinical oncology pharmacist experience with 1 year CPA experience
• Strong communication, organization, and tech skills
