The navigator will achieve streamlined patient care transitions and redirection to appropriate levels of care utilizing hospital and community resources to effectively educate and empower patients and their families.
The role requires adapting behavior to specific patient populations, conducting initial evaluations, identifying patient needs, and connecting patients to appropriate resources.
