The Nurse Case Manager will respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, and effective problem-solving abilities, both in a team environment as well as independently.
Responsible for assigned on caseload and act as the primary resource & contact for patients, physicians, Health Care Professionals and vendors within the designated area.
Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances.
