Job Summary

  • Provides clinically and evidence-based patient care coordination to support the delivery of safe, effective, high-quality, and efficient patient care.
  • Coordinates assessment, interdisciplinary discharge planning, and implementation of home healthcare and related services to assigned patients.
  • This role is PRN and will work (6) 8-hour days (Mon-Fri 8A-5P) per month, with training/orientation for 3 consecutive weekdays for 12 weeks.
