The Behavioral Health Specialist develops and maintains a comprehensive program of services for behavioral health care needs for patients and their families across the clinic setting.
This role serves as the mental health professional member of the multi-disciplinary healthcare team to improve the screening and management of social, emotional, and behavioral concerns and interrelated medical conditions for patients across the ambulatory care system.
The Behavioral Health Specialist follows an integrated care model within the ambulatory clinics to provide short-term counseling services to patients seeking mental health support and to assist patients with referrals to appropriate services in the community.
