  • This role serves as the mental health professional member of the multi-disciplinary healthcare team to improve the screening and management of social, emotional, and behavioral concerns and interrelated medical conditions for patients across the ambulatory care system.
  • The Behavioral Health Specialist follows an integrated care model within the ambulatory clinics to provide short-term counseling services to patients seeking mental health support and to assist patients with referrals to appropriate services in the community.
