Job Summary

  • The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Essential duties include meeting with administration, medical and nursing staff, and other related departments in planning social services, as well as assisting residents in achieving the highest practicable level of self-care, independence and well-being.
  • The role involves working with emotional problems, providing consultation to staff and community agencies, and assisting in discharge planning with appropriate agencies.
