Job Summary

  • As an independent contractor, the Nurse Practitioner will provide comprehensive primary care to residents as part of the interdisciplinary health care team.
  • The Nurse Practitioner will manage the care of residents by providing pharmacological, complementary and/or counseling interventions and provide outreach and transitional services to LTCH residents who return to the community.

Key Requirements

  • Master’s of Nursing or equivalent
  • Post-Master of Nursing (NP Field) Diploma
  • Registration with the College of Nurses of Ontario
  • 3-5 years of clinical nursing experience
  • Liability Insurance
  • WSIB Clearance Certificate
  • Vulnerable Position/Sector Screening (VPS)
  • TB Screening Complete
