The role provides oversight on facilities' weekly PDPM meetings and tracks effective utilization of PDPM tools for skilled patients.
The incumbent ensures accurate and timely completion of all MDS assessments including PPS Medicare, quarterly, annual, and significant change evaluations.
This position requires coordinating interdisciplinary participation to complete Minimum Data Sets according to regulatory time frames while maintaining confidentiality of resident care information.

Must-have
Accredited School of Nursing graduation
State Board of Nursing good standing
MDS assessment completion and submission
PDPM tool utilization tracking
Interdisciplinary care conference coordination
Nice-to-have
Bachelor of Science Degree in Nursing preferred
Long Term Care experience preferred
PCC software knowledge preferred
3 years supervisory experience preferred
Ability to incorporate new methods into practices
Key Requirements
