The Community Health Worker (CHW) will provide education, resources, and health education to patients, local community-based organizations, and community members.

Key Requirements
High School diploma/GED
Two (2) years of general experience
Enroll in certified CHW Training Program within 45 days
Completed certified CHW training program
Four (4) years of experience as a CHW
CHW certification from ARCHWA
Nine (9) years of experience as a CHW
