Skills & Requirements
Must-have
Bachelor's Degree in Nursing or Social Work
Registered Nurse license preferred
Two years clinical nursing experience
Knowledge of Medicare Medicaid programs
Effective communication with families and providers
Nice-to-have
Certified Case Manager (CCM) certificate
PCC Knowledge preferred
Intermediate computer skills
Sound judgement in high-volume environment
Ability to work independently
Key Requirements
Bachelor's Degree in Nursing or Social Work
RN license preferred; LVN/LPN acceptable
2 years clinical nursing experience
Maintain good standing with Department of Public Health
Complete required continuing education and licensing
