This role involves educating hospital staff about available services, assisting patients and families in understanding care options, and collaborating with discharge planners, physicians, and care teams to develop and implement patient-centered discharge plans.

Key Requirements
Clinical licensure appropriate to discipline (LPN/LVN) or Bachelor's Degree in Social Work
At least one year of full-time clinical experience
Current state professional license or certification
