Job Summary

  • The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs in accordance with current existing federal, state, and local standards.
  • You will assist residents in achieving the highest practicable level of self-care, independence and well-being while providing medically related social services to maintain their physical, mental and psychosocial well-being.
  • This role involves coordination with various departments and community agencies to ensure social service programs meet the needs of residents and their families.

  • Ability to lift and/or move up to 25 pounds
