Job Summary

This position requires approximately 80% field work, conducting in-home and facility visits to provide hands-on care coordination and member support.
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.

Key Requirements
Registered Nurse (RN) or LCSW/LCSW-A license
2 – 4 years of related experience
Bachelor's degree
Two (2) years of prior LTSS and/or HCBS coordination experience
Prior experience with social work, geriatrics, gerontology, pediatrics, or human services
