Job Summary

Key Requirements

  • High School diploma/GED
  • Two (2) years of general experience in community outreach, education, case management, or administration experience (Level I)
  • Four (4) years of experience as a CHW (Level II)
  • Nine (9) years of experience as a CHW (Level III)
  • Must enroll in a certified CHW Training Program within 45 days of employment (Level I)
  • Completed certified CHW training program (Level II)
  • Requires CHW certification from ARCHWA (Level III)
