Job Summary
Coordinates patient progress toward achieving quality and cost outcomes by mobilizing resources and managing the system in response to current variances.
Responsible for the coordination and completion of post-discharge needs for patients requiring home care or extended care facility services.
Communicates and collaborates with physicians, patients, and outside agencies in the development of the discharge plan in conjunction with the Care Management and Social Work teams for a safe and timely discharge.
