Job Summary

  • Provide trauma-informed, person-centered coordination of care and services for clients served by Hebrew SeniorLife’s Center for the Prevention of Elder Abuse and Neglect (CPEAN).
  • Assist with managing CPEAN’s elder abuse prevention phone line and email address, and perform intake, assessment, and discharge planning.
  • Conduct outreach to referral sources to foster collaborative relationships and continuously improve referral procedures.
