Job Summary

  • Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients utilizing Social Work Theory of Human Behavior and accepted methods of social work practice.
  • Collaborates with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life for patients while supporting quality assessment and improvement activities.
  • Assesses patient psychosocial status, provides counseling, facilitates transplant referrals, and works with insurance coordinators to address patient insurance and billing concerns.
