Lakeview Post Acute is seeking a Full-Time MDS Assistant (LVN) for a remote position responsible for overseeing MDS assessments, monitoring patient data, and providing training to new MDS Coordinators. The ideal candidate should possess a nursing degree, supervisory experience, and strong analytical skills

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
  • Assists disciplines in formulating and revising care plans, evaluates comprehensiveness and individuality, and is responsible for all level of care changes within the facility.
