Key Requirements

  • Doctorate in Pharmacy (PharmD)
  • Completion of PGY1 residency or 3 years health-system experience
  • Board certification through BPS within 24 months if not currently certified
  • Valid pharmacist licensure in Arizona
  • AZ Board of Pharmacy Wallet Card at time of hire
