Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, including emotional support and assistance with health care decisions.
Assists residents with personal shopping, inventory of belongings, coordination of response to missing items, securing prosthetics, and making outpatient/ancillary appointments.
