Job Summary
Develop individualized care plans by working closely with residents and their families to support them in achieving the highest level of independence and well-being.
Provide medically related social services to enhance residents’ physical, mental, and psychosocial health, including assessing needs, offering counseling, and assisting with healthcare decision-making.
Coordinate discharge planning with interdisciplinary teams and outside agencies, connect residents with community resources, and schedule outpatient appointments.
