  • The primary purpose of your job position is to oversee the facility to ensure assessments are completed per guidelines with a focus on the resident assessment process and plan of care.
  • You will provide oversight on weekly PDPM meetings, track facility utilization of PDPM tools, and provide data analysis and clinical recommendations for optimum outcomes.
  • The role includes coordinating interdisciplinary participation in MDS completion, conducting resident care conferences, and ensuring compliance with state and federal regulations.

Key Requirements

  • Graduation from Accredited School of Nursing
  • Bachelor of Science Degree in Nursing preferred
  • Good standing with State Board of Nursing
  • Maintain continuing education/licensing
  • 3 years supervisory experience preferred
  • Long Term Care experience preferred
