Job Summary

  • The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
  • Employees are responsible for reporting all changes in resident conditions, accidents, and suspected violations of privacy to the Health and Wellness Director immediately.

Must-have

  • Assist residents with daily hygiene functions
  • Monitor and report changes in resident condition
  • Maintain confidentiality of protected health information
  • Perform lifting and moving of residents up to 25 pounds
  • Complete documentation for incidents and care notes

Key Requirements

  • High school education or equivalent required
  • Proficiency in Microsoft Suite products required
