Key Requirements

  • Doctoral degree from accredited school of medicine or osteopathic medicine
  • Completed approved U.S. Residency/Fellowship in specified specialties
  • Current unrestricted board certification from ABFM, ABIM, or ABEM
  • Must pass pre-employment background check and fingerprint clearance
  • Maintain current DEA certification and NPI number upon hire
