  • The primary purpose of this job is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs to assure that the medically related emotional and social needs of the resident are met on an individual basis.
  • This position involves coordinating social service activities with other departments, providing consultation to staff and community agencies, and assisting residents in achieving the highest practicable level of self-care, independence, and well-being.
  • The role requires maintaining confidentiality of all resident care information and reporting any suspected incidents of fraud or unauthorized disclosure.
