The Patient Navigator serves as a liaison between patients, healthcare providers, and community resources to coordinate care and reduce barriers to treatment.
Candidates must possess strong organizational skills to track appointments, maintain detailed documentation, and advocate for patient needs within the interdisciplinary team.
