Support therapy departments with ROM exercises
Provide personal hygiene assistance
Coordinate social services for residents
Complete documentation and reporting
Maintain confidentiality of resident information

Strong communication and observation skills
Knowledge of PCC charting software
Ability to work in interdisciplinary teams
Supportive and positive work environment
Ability to assist in emergency evacuations
Key Requirements
Valid CNA license in California
Active RNA certification
Current CPR certification
High school diploma or equivalent
1 year long-term care experience preferred
Associate’s or Bachelor’s degree in Social Services preferred
