Job Summary

  • You will work on a multi-disciplinary team to build relationships and make an impact on members during important times in their lives.
  • This role provides comprehensive care coordination for members with high-risk, chronic complex conditions, ensuring adherence to the plan of care and facilitating options and services.
  • This is a Hybrid opportunity, working Monday - Friday, 8AM to 5PM, with no call required and some travel within your local territory (less than 25%).
