** Arbor Post Acute is seeking a Licensed Practical Nurse (LPN) to oversee the accurate completion of Minimum Data Set (MDS) assessments and to provide training and support to new MDS coordinators. The ideal candidate will have experience in long-term care, strong supervisory skills, and proficiency in data management tools. **

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
  • Schedules and conducts resident care conferences in compliance with state and federal regulations and assists disciplines in formulating and revising care plans.
